What Happens to Your Teeth if You Ignore a Sleep Breathing Problem?

😴 The Mouth Breathing Problem
During apnoeic episodes, the body instinctively switches to mouth breathing to restore airflow.
This causes a sustained reduction in salivary flow — a process that has been repeatedly demonstrated across two decades of research.
Saliva is not simply moisture. It performs several critical protective functions:
Buffers acid to protect enamel from erosion
Washes away food debris that feeds decay-causing bacteria
Acts as an antimicrobial agent against oral infection
Lubricates soft tissues and helps maintain mucosal integrity
When saliva production is impaired in OSA patients, the risks of dental caries, periodontal disease, and oral infections including candidiasis increase significantly.
🦷 Tooth Grinding (Bruxism)
Some authors have suggested that as many as one in two OSA patients may have bruxism
— often without knowing it. This nocturnal grinding is thought to represent a physiological attempt to reopen the collapsing airway.
The dental consequences are significant:
Progressive tooth wear and flattening of the biting surfaces
Fractures and cracks in otherwise sound teeth
Tooth sensitivity from exposed dentine
Increased periodontal stress, accelerating bone loss
🦠 Gum Disease
Chronic inflammation resulting from untreated OSA contributes to the progression of periodontal disease, worsening both systemic and oral health.
The intermittent drops in oxygen that characterise untreated OSA trigger an inflammatory cascade throughout the body — and
the gum tissues are not exempt from this, with OSA patients demonstrating deeper periodontal pockets, clinical attachment loss, and alveolar bone degeneration at higher rates than the general population.
🔄 A Vicious Cycle
Untreated sleep-disordered breathing creates a self-reinforcing cycle:
dry mouth worsens dental caries and periodontal disease; periodontal disease amplifies systemic inflammation; systemic inflammation worsens OSA severity.
The longer the condition goes undiagnosed, the more entrenched — and costly — the dental consequences become.
